OSHA's Refinery NEP – What's it all About?

The Occupational Health and Safety Administration (OSHA) issued an instruction (CPL-03-004: Petroleum Refinery Process Safety Management National Emphasis Program) effective June 7, 2007. The instruction requires OSHA to inspect virtually all refineries in SIC -2911 or NAICS – 324110 to verify employer's compliance with the PSM standard (29 CFR 1910.119).
This session is designed to provide attendees with the following:
- An understanding of the scope and application of OSHA's CPL-03-004
- A brief review of the incidents that led to the issuing of OSHA's CPL-03-004
- An overview of the specific auditing techniques to be used by the OSHA compliance officers during their inspections of refineries.

Real Estate Transactions for the Safety Professional

This presentation is an introduction to real estate transaction due diligence for the safety professional. This session includes review of relevant ASTM standards, US EPA "All Appropriate Inquiry" (AAI) final rule, innocent landowner and related defenses against Superfund liability, and the role of the safety professional on the due diligence team. The safety professional may be called upon by top management to organize, evaluate or conduct a real estate assessment, often with financing or other critical business negotiation issues on the line. The presentation will include a discussion of common pitfalls and how to avoid them.

“How Safe Are We? Approaches to Hazard Recognition”

Major incidents just don't happen. They are usually the result of multiple failures in a safety system and in particular failure to recognize hazards. Review of case studies and associated causal factors are a tool by which Hazard Recognition Teams can learn to better identify the types of hazards that are causing or likely to causes accidents or incidents. Hazard recognition teams can help to improve a safety programs overall performance when they understand why incidents occur and utilize this knowledge to identify hazards in the workplace.

Anatomy of a Disaster: Explosion at BP Texas City Refinery
U.S. Chemical Safety and Hazard Investigation Board Video Presentation
Three years after the explosion that killed 15 workers and injured 180 others at the BP Texas City refinery, the U.S. Chemical Safety Board (CSB) today released a new, comprehensive 56-minute video that describes the causes of the accident and key safety lessons. The safety video includes a new nine-minute 3-D computer animation of the sequence of events that led to the explosion, as well as sections describing BP's safety culture, the human factor safety issues that contributed to the accident, and the importance of safe equipment design and trailer siting. The video also features interviews with key members of the CSB investigative team, who completed the 341-page public report on the causes of the accident approved by the Board at a public meeting in Texas City on March 20, 2007. In the video, CSB Board Member William Wright discusses the Board's safety recommendations from the accident and key safety lessons from the Board's investigation.
